Each applicant must bring TWO (2) forms of indentification:
*Certified Birth Certificate with Official Seal
*Current Driver's License or State Photo Identification Card

ALL APPLICANTS MUST APPEAR IN PERSON (INCLUDING MINORS)
14 YEARS AND UNDER MUST APPEAR WITH BOTH PARENTS.
IF ONE IS NOT AVAILABLE, A NOTORIZED AUTHORIZATION
FROM THE ABSENT PARENT IS REQUIRED.
